My principal research interest is management of poisoning and adverse drug reactions to antidotes.
My research aims to optimise acetylcysteine treatment regimens for paracetamol poisoning to reduce adverse reactions without compromising efficacy. I devised using pharmacokinetic modelling a shorter 12h acetylcysteine regimen. I was the Principal Investigator in Newcastle for the Scottish and Newcastle Anti-Emetic Pre-Treatment in Paracetamol Poisoning (SNAP) clinical trial which showed this shorter 12h regimen of acetylcysteine reduced the incidence of vomiting and anaphylactoid reactions (published in Lancet 2013).
I subsequently devised a clinical protocol (Scottish and Newcastle Acetylcysteine protocol) which was implemented in clinical practice in several hospitals in the UK and was audited for effectiveness compared to the conventional 21h acetylcysteine regimen. This audit showed that the SNAP regimen was as effective as the conventional 21h regimen and was associated with fewer adverse reactions.(published 2019)
In May 2023, the Royal College of Emergency Medicine (RCEM), National Poisons Information Service (NPIS) and British Association for the studyof the Liver (BASL) issued a joint position statement recommending the SNAP regimen as the standard of care for treatment of paracetamol overdose in the UK.
