I have worked as a Consultant Neonatal Paediatrician at the RVI, Newcastle since 2003 (qual.1990), and have held an Honorary University post since appointment.

Interdisciplinary research themes
(1) Nutrition & gut function. We collaborate with academics at a national and international level including universities in Newcastle, Northumbria, Tees, York, Liverpool, Oxford, Amsterdam and Copenhagen. I co-founded the UK neonatal nutrition network (N3) in 2003 and from this developed collaborations that led to successful large scale NIHR Health Technology Assessment (HTA) funded nutrition trials in preterm infants (£5.8million funding 2012-2020). These large, successful trials recruited >5000 premature infants from over 30 hospitals, making them the largest interventional nutritional studies ever completed in this population.
Our published data show necrotising enterocolitis, a devastating gut inflammatory condition, is the commonest cause of death after the first week of life in preterm infants, with over 50% either dying or surviving with brain impairments resulting in cerebral palsy. We collaborate with teams at Newcastle, Northumbria and Liverpool to conduct studies that will improve understanding of gut function and the role of gut microbiota, metabolites and tissue response. I am Chief Investigator (2016-2020) on an NIHR (Efficacy & Mechanistic Evaluation) funded study (MAGPIE £499,000 Newcastle Hospitals Sponsor). This study builds on our developing work with applied immunobiology teams, to explore gut cell function and immune response (Kirby, Hambleton, Lamb, Institute of Cellular Medicine, ICM)
(2) Growth & metabolic outcome. I established the Newcastle Preterm Growth Study which has tracked the growth of preterm infants born in 1993-97 until late adolescence. This work determines the long-term effects of early life nutrition on growth and metabolic function (Prof Trenell, Dr Cheetham), as well as epigenetic correlates (Prof Relton). Our cohort is one of the most comprehensive and unique long-term growth studies in premature infants worldwide. I also lead work focused on the impact of breast-feeding, and am Chief Investigator (2017-2020) on an investigator led, interventional randomised study of an exclusive human milk diet (INDIGO £808,000 Newcastle Hospitals Sponsor) and funded by a US bioscience company.
(3) Reproductive loss. Unfortunately, death in premature infants is not uncommon, and my clinical insight and access have allowed me to develop collaborations with colleagues in IHS (Prof Rankin) and the School of Geography, Politics and Sociology (Dr Graham). We have secured >£200,000 of charitable and other funds to conduct unique, qualitative studies with parents and staff to better understand the lived experiences of parents who suffer a reproductive loss, focusing on neonatal death and the withdrawal of life sustaining intensive care. (4) Movement analysis, brain & neuroscience. We collaborate with Dr Basu (IoN & NIHR fellowship) and an academic physiotherapist to support innovative studies exploring the later life physical consequences (e.g. cerebral palsy) of early life events that result in brain damage. We secured funding and successfully recruited infants to pilot intervention trials, and conduct innovative studies exploring neuro-development using movement sensors. We are successful co-applicants on a NIHR HTA funded (£268,000; 2015-2018) study that will determine the childhood outcome following an in-utero diagnosis of a congenital brain anomaly (MERIDIAN).
Research summary. Our research programme is successful, competitively funded (~£8M total) and thematically broad-based. It involves multi-disciplinary teams, different disciplines (five institutes in FMS at Newcastle), national and international collaborations, and utilises a variety of methods (trials, cohorts, basic science and qualitative studies).
Innovation & impact
- Our trials provide high quality evidence that determine best practice in neonatal nutrition, and will help reduce mortality and morbidity from serious newborn diseases. Our Cochrane Systematic Reviews help synthesise the best available evidence for clinical practice.
- Our cohort studies in preterm infants recruited >500 preterm infants locally from whom we collected >20,000 samples of stool, urine and blood. We have established the unique Great North Neonatal Biobank (Newcastle University). This will support a series of basic scientific studies of gut health over the next five years.
- Our qualitative research improves the experience of parents who suffer the devastation of neonatal death. We secured Academic Health Science Network and charitable funding to translate our qualitative research findings into improving care, working with advocacy organisation in the UK and Europe. We developed guidelines and education packages translated into five languages, and shared with >350 hospitals worldwide. Our innovative and ethnographic film work resulted in the Butterfly project: this focuses on the unique (but not uncommon) situation of co-twin loss (where one baby of a twin pair dies and one survives).
